However, a reference range for the MI in healthy children and the diagnostic significance of MI values falling outside the normal range in other anemias have not been established.</p> <p><bold>Aim:</bold> to evaluate the potential utility of the MI for the differential diagnosis of anemia in children.</p> <p><bold>Materials and methods.</bold> In the first stage of this two-stage retrospective study conducted at a pediatric outpatient clinic in Orel, the MI was calculated for 619 healthy children aged between 1 and 18 years. Age-specific reference ranges were determined as the mean ± 2 standard deviations. In the second stage of the study conducted both at a pediatric outpatient clinic and the regional specialized department of pediatric oncology and hematology, the MI was calculated for 153 children of the same age with signs of anemia on initial complete blood count at the onset of various hematologic diseases. We assessed the sensitivity, specificity, and predictive value of MI values outside the reference ranges established in the first stage of the study for the screening of these diseases.</p> <p><bold>Results.</bold> In healthy children aged &gt;1 year, MI values did not depend on age (<italic>p</italic> &gt; 0.86) or gender (<italic>p</italic> &gt; 0.34); the reference range was 14.1–22.0, with a mean of 18.0. MI values in all types of blood disorders in children presenting with anemia fell significantly outside the reference ranges (<italic>p</italic> &lt; 0.001). In β-thalassemia patients, MI values were below the reference range (the median (Q1; Q3): 11.0 (10.0; 11.0)); in iron-deficiency anemia patients, MI values fell within the reference range (14.6 (11.5; 18.6)). Children with other hemolytic anemias and acute leukemia had MI values above the reference range (29.0 (23.5; 37.0) and 32.0 (27.0; 51.0), respectively). The highest MI values were observed in patients with vitamin B<sub>12</sub>/folate deficiency (59.0 (41.5; 67.5)) and bone marrow aplasia (59.5 (47.0; 92.3)). An MI threshold of &lt;13 for β-thalassemia screening showed a sensitivity of 90.9% (95% confidence interval (CI) 58.7–99.8) and a specificity of 66.7% (95% CI 54.6–77.3), respectively. An MI threshold of &gt;23 for other rare anemias showed a sensitivity of 85.7% (95% CI 75.3–92.9) and a specificity of 91.7% (95% CI 82.7–96.9), respectively, with a predictive value of a negative result of &gt;99.9%.</p> <p><bold>Conclusion.</bold> In children older than 1 year, the MI has high diagnostic potential not only for β-thalassemia screening (an MI of &lt;13), but also for detecting other rare anemias (an MI of &gt;23), including those associated with life-threatening diseases such as acute leukemia and bone marrow aplasia. Routine calculation of the MI in children with decreased hemoglobin levels in resource-limited primary healthcare facilities would help early detection of severe and rare hematologic disorders in children.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение.</bold> Индекс Ментцера (Mentzer Index, MI), определяемый как отношение среднего объема эритроцитов к их числу, еще с 1973 г. успешно используется для скрининга β-талассемии по результатам общего анализа крови. Однако референсный диапазон (РД) MI для здоровых детей и диагностическая значимость его отклонений при других анемиях не установлены.</p> <p><bold>Цель исследования</bold> – оценить возможность применения MI для дифференциальной диагностики анемических состояний в детском возрасте.</p> <p><bold>Материалы и методы.</bold> На 1-м этапе двухэтапного ретроспективного исследования на базе детской поликлиники г. Орла был рассчитан MI у 619 здоровых детей в возрасте от 1 года до 18 лет (I группа здоровья) и определены его возрастные РД по методу 2σ-отклонений (среднее значение ± 2 стандартных отклонения). На 2-м этапе в условиях детской поликлиники и специализированного областного отделения детской онкологии и гематологии был рассчитан MI у 153 детей того же возраста с признаками анемии в первичной гемограмме на момент дебюта различных заболеваний крови. Оценивались чувствительность, специфичность и прогностическая значимость отклонений MI за пределы установленных на 1-м этапе РД для скрининга данных заболеваний.</p> <p><bold>Результаты.</bold> У здоровых детей старше 1 года величина MI не зависела от возраста (<italic>p</italic> &gt; 0,86) и пола (<italic>p</italic> &gt; 0,34), РД составил 14,1–22,0 со средним значением 18,0. Величины MI при всех типах патологии крови у детей, протекающих с анемией, достоверно (<italic>p</italic> &lt; 0,001) выходили за пределы РД. Величины MI ниже РД зарегистрированы при β-талассемии (медиана (Q1; Q3) – 11,0 (10,0; 11,0)), находились в рамках РД при железодефицитной анемии (14,6 (11,5; 18,6)), оказались выше РД при других гемолитических анемиях (29,0 (23,5; 37,0)) и острых лейкозах (32,0 (27,0; 51,0)), были максимальными при дефиците витамина В<sub>12</sub>/фолатов (59,0 (41,5; 67,5)) и аплазиях кроветворения (59,5 (47,0; 92,3)). Порог MI &lt;13 для скрининга β-талассемии показал чувствительность и специфичность на уровнях 90,9% (95% доверительный интервал (ДИ) 58,7–99,8) и 66,7% (95% ДИ 54,6–77,3) соответственно, порог MI &gt;23 для скрининга других редких анемий – 85,7% (95% ДИ 75,3–92,9) и 91,7% (95%ДИ 82,7–96,9) соответственно с прогностической значимостью отрицательного результата &gt;99,9%.</p> <p><bold>Заключение.</bold> У детей старше 1 года MI обладает высоким диагностическим потенциалом не только для скрининга β-талассемии при величине &lt;13, но и для выявления других редких анемий при значении &gt;23, в том числе ассоциированных с жизнеугрожающими заболеваниями (острые лейкозы, аплазии кроветворения).
