Despite current advances in pediatric oncology, there is a high need, both in Russia and abroad, to optimize resources for the organization of medical care for children with cancer taking into account a multidisciplinary approach, standardization of patient referral procedures, implementation of resource-saving technologies, and personalized treatment strategies.</p> <p><bold>Aim:</bold> to evaluate the current organization of medical care for children with malignant neoplasms in the Stavropol Krai, identify its strengths and weaknesses, identify future directions for development, and propose ways to improve this system.</p> <p><bold>Materials and methods.</bold> We performed a retrospective analysis of epidemiological data (incidence, disability, and mortality rates) and resources of pediatric oncology and hematology service based on official statistical reports for 2015–2024 and materials from an on-site audit of the main regional healthcare facilities conducted in November 2024. We analyzed epidemiological trends in the Stavropol Krai over the past years and compared them with national data, and assessed organizational and methodological aspects of the service.</p> <p><bold>Results.</bold> In 2024, the incidence of cancer in children was 12.8 per 100 000 (76 new cases). Over 2021–2024, the trend was fluctuating, with a minimum of 10.8 per 100 000 in 2021 and a maximum of 14.2 per 100 000 in 2022. There is a steady increase in the number of new cases of cancer-related disability among children: from 31 cases in 2021 to 38 cases in 2024. Specialized care is centralized at the Regional Children’s Clinical Hospital and organized according to a three-tier system, with active referral of patients to federal centers. Times to diagnosis and treatment in case of acute leukemia are short (1–3 days and 1–2 days, respectively), in bone tumors – 5–7 days; however, there are still serious delays in the diagnosis and treatment of solid tumors (up to 10 days in case of lymphomas, 7–21 days in case of central nervous system tumors).</p> <p><bold>Conclusion.</bold> The strengths of the pediatric oncology/hematology service in the Stavropol Krai include the centralization of medical care, a well-developed regional health infrastructure, high staffing levels of pediatric hematologists, as well as prompt patient referral and timely treatment initiation for children with acute leukemia. However, there are still some issues to address, such as a shortage of pediatric oncologists and hematologist-oncologists, delays in the diagnosis and treatment of solid tumors, the need for further development of health infrastructure, standardization of patient referral pathways, and treatment outcome monitoring.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение.</bold> Злокачественные новообразования (ЗНО) у детей – одна из ведущих причин детской инвалидизации и смертности во всем мире. Несмотря на достижения современной детской онкологии, в России и за рубежом сохраняется высокая потребность в оптимизации ресурсов при организации медицинской помощи детям с ЗНО с учетом мультидисциплинарного подхода, стандартизации маршрутизации пациентов, внедрения ресурсосберегающих технологий и персонификации методов лечения.</p> <p><bold>Цель исследования</bold> – оценить текущую организационную модель оказания медицинской помощи детям с ЗНО в Ставропольском крае, выявить ее сильные и проблемные стороны, определить перспективы развития и предложить пути оптимизации системы.</p> <p><bold>Материалы и методы.</bold> Проведен ретроспективный анализ показателей эпидемиологии (заболеваемость, инвалидность, смертность) и ресурсов службы детской онкологии-гематологии на основе официальных статистических отчетов за 2015–2024 гг., материалов выездного аудита ключевых учреждений Ставропольского края в ноябре 2024 г. Выполнены анализ динамики показателей за последние годы в Ставропольском крае и в сравнении с общероссийскими данными и оценка организационно-методических аспектов работы службы.</p> <p><bold>Результаты.</bold> Заболеваемость ЗНО у детей в 2024 г. составила 12,8 на 100 тыс. детского населения (76 новых случаев), динамика показателя за 2021–2024 гг. носила волнообразный характер: минимум – 10,8 на 100 тыс. в 2021 г., максимум – 14,2 на 100 тыс. в 2022 г. Отмечается устойчивый рост числа детей, впервые признанных инвалидами по причине ЗНО: с 31 случая в 2021 г. до 38 случаев в 2024 г. Специализированная помощь централизована на базе Краевой детской клинической больницы и организована по трехуровневому принципу с активным направлением пациентов в федеральные центры. При острых лейкозах обеспечены короткие сроки диагностики и начала терапии (1–3 и 1–2 дня соответственно), при опухолях костей – 5–7 дней, тогда как при солидных опухолях сохраняются значимые задержки (лимфомы – до 10 дней, опухоли центральной нервной системы – от 7 до 21 дня).</p> <p><bold>Заключение.</bold> Отмечены сильные стороны службы детской онкологии-гематологии в Ставропольском крае (централизация помощи, развитая инфраструктура краевого уровня, высокая обеспеченность детскими гематологами, быстрая маршрутизация и начало терапии при острых лейкозах), однако сохраняются проблемные зоны: дефицит детских онкологов и онкологов-гематологов, задержки в диагностике и лечении солидных опухолей, необходимость дальнейшего дооснащения инфраструктуры и стандартизации маршрутизации и мониторинга исходов.</p></trans-abstract><kwd-group xml:lang="en"><kwd>pediatric oncology/hematology</kwd><kwd>Stavropol Krai</kwd><kwd>incidence</kwd><kwd>patient referral</kwd><kwd>territorial program of state guarantees</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>детская онкология-гематология</kwd><kwd>Ставропольский край</kwd><kwd>заболеваемость</kwd><kwd>маршрутизация пациентов</kwd><kwd>территориальная программа государственных гарантий</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Ward Z.J., Yeh J.M., Bhakta N., Lindsay Frazier A., Girardi F., Atun R.
