The possible causes of recurrence are considered to be incompleteness of resection and histological misinterpretation of primary tumor, non-removal or incomplete resection of coccyx in teratomas of sacrococcygeal region (SCR). Presence of yolk sac tumor (YST) foci in the tumoral tissue and intraoperative tumoral rupture and spillage are regarded as additional risk factors.</p> <p><bold>The aim of study</bold> – to analyze the cases of recurrence of neonatal teratomas based on own 20 years’ experience.</p> <p><bold>Materials and methods.</bold> Medical charts from one surgical and one oncological clinic in St-Petersburg were investigated during a period of 2005–2024. There were in total 77 newborns with GCT of sacrococcygeal (<italic>n</italic> = 63), abdominal and retroperitoneal (<italic>n</italic> = 5), cervical (<italic>n</italic> = 4), and other (<italic>n</italic> = 5) localizations. Histologically 57 were mature and 20 – immature teratomas, 7 of them comprising the YST component. All of them underwent removal of teratoma during their neonatal period, 51 being subsequently sent to pediatric oncologist’s follow-up. Our investigation included only patients with known catamnesis (<italic>n</italic> = 64).</p> <p><bold>Results.</bold> Relapses occurred in 8 (12.5%) children after removal of SCR (<italic>n</italic> = 7) and cervical (<italic>n</italic> = 1) teratomas, of following histological types: mature (<italic>n</italic> = 4), immature Grade 3 (<italic>n</italic> = 2), immature Grade 3 with YST (<italic>n</italic> = 2). The relapses were first suspected within 2–18 months based on control ultrasound data (<italic>n</italic> = 3) or 1.4–12.6-fold elevation of serum alpha-fetoprotein (<italic>n</italic> = 5), with following confirmation by computed tomography or magnetic resonance imaging. All the recurrent nodes were small-sized (1.7–4.3 cm), however, 1 of those patients had already developed metastases in lungs, liver, and bones by that time, requiring administration of polychemotherapy. Seven of 8 children underwent repeated surgery, in 5 of them the removed node was presented by YST: in pure form (<italic>n</italic> = 4) or as a component of immature teratoma Grade 3 (<italic>n</italic> = 1), which deserved adjuvant polychemotherapy according to MAKEI 96/05 protocols. Two patients with recurrence in form of YST died, whereas the other 6 have a favorable outcome (the follow-up period being 7–18 years). The assessment of possible causes of relapses allowed to rule out the incomplete coccyx resection (based on control computed tomography), as well as histological misinterpretation of primary tumor (based on additional review of material at a reference-center) in all represented cases. In 2 children a tumoral rupture during the initial surgery was noted. We performed a statistical analysis of prognostic role of this factor (as well as the size, structure, histological type of primary tumor, initial serum alpha-fetoprotein level, and some others) by means of StatTech v. 4.9.5 program (with Pearson's chi-square test and Fisher’s exact test as used methods). There occurred no correlation between those variables and recurrence risk (<italic>p</italic> = 0.159–1.000, differences being statistically significant at <italic>p</italic> &lt; 0.05)</p> <p><bold>Conclusion.</bold> The major cause of teratomas’ recurrence in the investigated cohort should be considered non-radicality of their surgical removal which occurred unintentionally, since macroscopically the tumor resection was estimated as complete (radical) in all cases. The accuracy of surgical radicality assessment is achieved by postoperative instrumental and laboratory control performed thoroughly and in time, as well as by pathologist’s description of resection margins. In more than a half cases the recurrent node tissue contained YST – independently on the histological type of initial tumor. Therefore, all types of GCT including mature teratoma do require prolonged follow-up by pediatric oncologist after surgical removal, whereas any recurrent node (even small-sized) should be regarded as potentially malignant, deserving most active approach.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение.</bold> Тератомы – самый распространенный вариант герминогенно-клеточных опухолей (ГКО) у новорожденных, они требуют первично хирургического лечения, имеют в целом благоприятный прогноз, однако примерно в 11% случаев способны рецидивировать. Возможными причинами рецидивов считаются неполнота резекции и ошибки гистологической интерпретации первичной опухоли, неудаление или неполное удаление копчика при тератомах крестцово-копчиковой области (ККО). В качестве дополнительных факторов риска рассматриваются присутствие в образовании очагов опухоли желточного мешка (ОЖМ) и факт интраоперационного разрыва опухолевой ткани.</p> <p><bold>Цель работы</bold> – проанализировать случаи рецидивов тератом у новорожденных из собственной практики за 20 лет.</p> <p><bold>Материалы и методы.</bold> Изучены истории болезни пациентов хирургического и онкологического стационаров Санкт-Петербурга с 2005 по 2024 г. Всего пролечены 77 новорожденных с ГКО: 63 – ККО, 5 – брюшной и забрюшинной, 4 – шейной, 5 – других локализаций. Гистологически 57 были зрелыми, 20 – незрелыми тератомами, в 7 из них присутствовал компонент ОЖМ. Всем выполнено удаление тератомы в периоде новорожденности, после чего 51 ребенок наблюдался детским онкологом. В исследование взяты только пациенты с известным катамнезом (<italic>n</italic> = 64).</p> <p><bold>Результаты.</bold> Рецидивы отмечены у 8 (12,5%) детей после удаления тератом ККО (<italic>n</italic> = 7) и шеи (<italic>n</italic> = 1), гистологические варианты: зрелая (<italic>n</italic> = 4), незрелая Grade 3 (<italic>n</italic> = 2), незрелая Grade 3 c ОЖМ (<italic>n</italic> = 2). Рецидивы были заподозрены через 2–18 мес на основании данных контрольного ультразвукового исследования (<italic>n</italic> = 3) или повышения уровня альфа-фетопротеина крови в 1,4–12,6 раза (<italic>n</italic><italic> </italic>= 5) с последующим подтверждением с помощью компьютерной или магнитно-резонансной томографии. Все рецидивные узлы в ККО были небольшими (1,7–4,3 см), однако у 1 пациента при этом уже имелись метастазы в легких, печени и костях, ему была назначена полихимиотерапия. Семь из 8 детей были оперированы повторно, у 5 из них удаленный узел был представлен ОЖМ: в чистом виде (<italic>n</italic> = 4) или в структуре незрелой тератомы Grade 3 (<italic>n</italic> = 1), что потребовало проведения адъювантной полихимиотерапии по протоколам MAKEI 96/05. Два пациента с рецидивами в виде ОЖМ погибли, у остальных 6 исход благоприятный (сроки наблюдения 7–18 лет). Анализ возможных причин рецидивов позволил исключить неполноту удаления копчика (на основании контрольных компьютерных томограмм) и ошибки в интерпретации гистологического варианта первичной опухоли (по данным пересмотра материала в референс-центре) во всех представляемых наблюдениях. У 2 детей было зафиксировано нарушение целостности структуры опухоли во время первой операции. Прогностическая роль этого фактора (а также размеров, структуры, гистологического варианта изначальной опухоли, исходного уровня альфа-фетопротеина крови и ряда других) была оценена статистически при помощи программы StatTech v. 4.9.5 (методы: Хи-квадрат Пирсона и точный критерий Фишера). Достоверной зависимости риска рецидива от этих показателей не получено (<italic>p</italic> = 0,159–1,000 при статистической значимости <italic>p</italic> &lt; 0,05).</p> <p><bold>Заключение.</bold> Основной причиной рецидива тератом в исследованной группе следует считать нерадикальность их хирургического удаления, оказавшуюся непреднамеренной: макроскопически иссечение опухоли было во всех случаях расценено как полное (радикальное). Объективизация оценки радикальности операции достигается проведением тщательного и своевременного инструментального и лабораторного контроля в послеоперационном периоде, а также описанием краев резекции врачом-морфологом. Более чем в половине случаев в ткани рецидивного узла присутствовала ОЖМ, независимо от гистологического типа первичной опухоли.
