Despite evidence supporting the efficacy of this approach, its tolerability, technical feasibility, and toxicity profile in the pediatric population remain subjects of ongoing investigation.</p> <p><bold>Objective</bold> – to analyze the tolerability, toxicity, and technical feasibility of tandem HDCT with auto-HSCT in children with high-risk SMN, including a comparative assessment against single auto-HSCT.</p> <p><bold>Materials and methods.</bold><bold> </bold>A retrospective-prospective single-center study was conducted, enrolling 115 pediatric patients (median age 48 (range – 7–215) months) with high-risk SMN who underwent HDCT with auto-HSCT. The study group comprised 20 patients who received tandem HDCT with auto-HSCT: 14 with germ cell tumors, 4 with neuroblastoma, 1 with hepatoblastoma, and 1 with sialoblastoma. The historical control group (<italic>n</italic> = 95) included 74 patients with neuroblastoma, 16 with Ewing sarcoma, and 5 with germ cell tumors, all of whom received single auto-HSCT. A graft of satisfactory quality was successfully collected from all patients in the study group. All patients received a conditioning regimen according to the underlying disease protocol, performance status, and baseline characteristics. Therapy toxicity, outcomes, and transplant-related mortality (TRM) were evaluated.</p> <p><bold>Results.</bold> Hematopoietic recovery was achieved in all patients in the study group after both transplantation stages, the median time to neutrophil engraftment was 10.5 and 11.5 days after auto-HSCT №1 and auto-HSCT №2, respectively. Grade III–IV toxic complications were recorded in 40.0% of patients after auto-HSCT №1 and in 90.0% after auto-HSCT №2 (<italic>p</italic> = 0.001), which was comparable to the historical control group (81.1%). TRM in the study group was 0%. At the time of last follow-up, 17 (85.0%) patients in the study group were alive, of whom 16 (80.0%) were in remission; in the historical control group, 74 (77.9%) patients were alive, of whom 70 (73.7%) were in remission.</p> <p><bold>Conclusion.</bold> Tandem HDCT with auto-HSCT is a technically feasible consolidation strategy in children with high-risk SMN, with an acceptable tolerability and toxicity profile. The absence of TRM and a manageable toxicity profile comparable to single auto-HSCT support the expansion of indications for this approach. Further multicenter studies are needed to evaluate long-term outcomes and optimize patient selection criteria.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение.</bold> Тандемная высокодозная полихимиотерапия (ВДПХТ) с последующей аутологичной трансплантацией гемопоэтических стволовых клеток (ауто-ТГСК) является методом интенсификации консолидирующей терапии у детей с рядом солидных злокачественных новообразований (ЗНО) группы высокого риска. Несмотря на ряд исследований, демонстрирующих эффективность данного метода лечения ЗНО, остаются актуальными вопросы оценки его переносимости, технической осуществимости и профиля токсичности при применении у детей.</p> <p><bold>Цель исследования</bold> – анализ переносимости, токсичности и технической осуществимости тандемной ВДПХТ с ауто-ТГСК у детей с солидными ЗНО высокого риска, в том числе в сравнении с одиночной ауто-ТГСК.</p> <p><bold>Материалы и методы.</bold> Проведено ретроспективно-проспективное одноцентровое исследование, в которое были включены 115 пациентов детского возраста (медиана возраста – 48 (7–215) месяцев) с солидными ЗНО группы высокого риска, которым была выполнена ВДПХТ с ауто-ТГСК. Основную группу составили 20 пациентов, получивших тандемную ВДПХТ с ауто-ТГСК: 14 пациентов с герминогенно-клеточными опухолями, 4 – с нейробластомой, 1 – с гепатобластомой, 1 – с сиалобластомой. В группу исторического контроля (<italic>n</italic> = 95) были включены 74 пациента с нейробластомой, 16 – с саркомой Юинга, 5 – с герминогенно-клеточными опухолями, получившие одиночную ауто-ТГСК. Для всех пациентов основной группы удалось заготовить трансплантат удовлетворительных характеристик. Все пациенты получили режим кондиционирования в зависимости от протокола лечения основного заболевания, соматического статуса и инициальных характеристик. Оценивалась токсичность терапии, исходы, летальность, связанная с трансплантацией.</p> <p><bold>Результаты.</bold> Восстановление гемопоэза наступило у всех пациентов основной группы после обоих этапов, медиана восстановления гранулоцитарного ростка составила 10,5 и 11,5 дней после ауто-ТГСК №1 и ауто-ТГСК №2 соответственно. Токсические осложнения III–IV степени зафиксированы у 40,0% пациентов после ауто-ТГСК №1 и у 90,0% – после ауто-ТГСК №2 (<italic>p</italic> = 0,001), что сопоставимо с группой исторического контроля (81,1%). Летальность, связанная с трансплантацией, в основной группе составила 0%. На момент последнего наблюдения в основной группе живы 17 (85,0%) пациентов, из них 16 (80,0%) – в ремиссии, в группе исторического контроля живы 74 (77,9%), из них 70 (73,7%) – в ремиссии.</p> <p><bold>Заключение.</bold><bold> </bold>Тандемная ВДПХТ с ауто-ТГСК является технически осуществимым методом консолидирующей терапии у детей с солидными ЗНО группы высокого риска с приемлемым уровнем переносимости и токсичности. Отсутствие случаев летальности, связанной с трансплантацией, и управляемый профиль токсичности, сопоставимый с одиночной ауто-ТГСК, обосновывают расширение показаний к данному методу.
