However, intensive multimodal treatment strategy is associated with a significantly increased risk of secondary malignancies. It is currently known that alkylating agents, topoisomerase II inhibitors, and platinum compounds induce treatment-related leukemia. This article presents a literature review and description of a clinical case of secondary acute myeloid leukemia (s-AML) developed 57 months after the initial diagnosis in a patient with intermediate-risk NB who received intensive first-line and post-relapse chemotherapy. The debut of s-AML required a differential diagnosis to rule out a relapse of NB. Parents gave their consent to use information about the child, including fotos, in the article.</p></abstract><trans-abstract xml:lang="ru"><p>Современные успехи детской онкологии улучшили показатели выживаемости пациентов с нейробластомой (НБ) группы промежуточного и высокого риска. В то же время интенсивная мультимодальная терапия приводит к увеличению частоты встречаемости вторичных злокачественных новообразований у выживших пациентов. В настоящее время известно, что алкилирующие агенты, ингибиторы топоизомеразы II и препараты платины могут приводить к развитию вторичного острого миелоидного лейкоза (ОМЛ). В данной статье представлены литературный обзор и описание клинического случая развития вторичного ОМЛ через 57 мес от установления первичного диагноза у пациента с НБ группы промежуточного риска, получавшего интенсивную химиотерапию в 1-й линии и в рецидиве заболевания. Дебют вторичного ОМЛ потребовал выполнения ряда диагностических исследований для исключения рецидива НБ.
