Whereas previously the incidence rate was 2.6 cases per million children, now it is 5.3 cases per million. For children diagnosed with HB, the current overall survival rate is approaching 80%. Brain metastases in HB are rare, and there are very few publications reporting long-term outcomes in such patients.</p> <p><bold>Case report.</bold> In this article, we present a clinical case report of a patient with very high-risk HB who received several lines of chemotherapy and repeated surgical treatment for pulmonary metastases followed by the development of a single intracerebral metastasis. After the removal of the metastasis, systemic therapy was continued which resulted in remission lasting 16 months. Although the overall prognosis in cases of CNS involvement is unfavorable, patients with a single lesion can achieve long-term survival with comprehensive treatment.</p> <p><bold>Conclusion.</bold> Despite the advances of modern medicine, outcomes in patients with brain metastases from malignant solid tumors remain unfavorable, and further research is needed to optimize treatment methods in order to improve long-term outcomes and increase survival rates.</p></abstract><trans-abstract xml:lang="ru"><p>В развитых странах заболеваемость гепатобластомой (ГБ) растет, если раньше частота встречаемости составляла 2,6 случая на 1 млн детского населения, то сейчас – 5,3 случая. Общая выживаемость детей с ГБ в настоящее время приближается к 80%. Метастазы в головной мозг при ГБ встречаются редко, публикаций в литературе с результатами отдаленных исходов у таких пациентов крайне мало.</p> <p><bold>Клинический случай.</bold> В данной работе представлено описание клинического случая пациента с ГБ группы очень высокого риска, получившего несколько линий химиотерапии и повторные хирургические вмешательства в связи с легочными метастазами с последующим развитием одиночного внутримозгового метастаза, после удаления которого продолжена системная терапия с достижением ремиссии длительностью 16 мес. Хотя в целом прогноз при поражении центральной нервной системы неблагоприятный, при обнаружении одного очага можно добиться длительного выживания с помощью комплексного лечения.</p> <p><bold>Заключение. </bold>Несмотря на возможности современной медицины, исходы при метастазах злокачественных солидных опухолей в головной мозг остаются неблагоприятными, необходимы дальнейшие исследования для оптимизации методов терапии в целях улучшения отдаленных исходов и повышения показателей выживаемости.</p></trans-abstract><kwd-group xml:lang="en"><kwd>hepatoblastoma</kwd><kwd>brain</kwd><kwd>intracranial metastases</kwd><kwd>children</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>гепатобластома</kwd><kwd>головной мозг</kwd><kwd>внутричерепные метастазы</kwd><kwd>дети</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Karalexi M.A., Servitzoglou M., Moschovi M., Moiseenko R., Bouka P., Ntzani E. et al.; NARECHEM-ST collaborative group; Moscow childhood liver tumor study group.
