The only curative treatment for patients with NBS is hematopoietic stem cell transplantation (HSCT) which is, however, associated with a high risk of endothelial toxicity and severe graft-versus-host disease. If previously HSCT was performed mainly in patients with oncologic complications, now recent statistical data and the implementation of neonatal screening support the necessity of early HSCT. A shift in the field of stem cell transplantation toward conditioning regimens aimed to reduce toxicity and risk of secondary malignancies may result in persistent mixed chimerism and insufficient antitumor effects in NBS patients with cancer. Currently, there are no standardized guidelines regarding the timing of HSCT, the selection of conditioning regimens or graft-versus-host disease prophylaxis in NBS.</p> <p><bold>Aim:</bold> to demonstrate the evolution of HSCT approaches in children with NBS at a single center and to perform a retrospective analysis of therapeutic effectiveness with the assessment of long-term outcomes.</p> <p>Materials and methods. Over a 23-year period, 9 children with NBS underwent HSCT at the Russian Children’s Clinical Hospital.</p> <p><bold>Results.</bold> The overall 10-year post-transplant survival rate was 0.67 (95% confidence interval 0.56–0.78). There were 3 transplant-related deaths. Most of the patients maintain mixed chimerism and require immunoglobulin replacement therapy.</p> <p><bold>Conclusion.</bold> Taking into account the current trends and advances, HSCT approaches for NBS patients have been improved leading to better treatment outcomes. However, given the specific features of the disorder, further research is still needed to develop rational therapeutic strategies.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение.</bold> Cиндром Ниймеген (СН) относится к группе врожденных патологий, характеризующихся спонтанной хромосомной нестабильностью, иммунодефицитом, предрасположенностью к онкологическим заболеваниям и повышенной чувствительностью к специфическим агентам, повреждающим ДНК. Единственным радикальным методом терапии таких пациентов остается трансплантация гемопоэтических стволовых клеток (ТГСК), ассоциированная с высокими рисками эндотелиальной токсичности и тяжелой реакцией «трансплантат против хозяина». Если ранее ТГСК проводилась в большинстве случаев при наличии онкологических осложнений, то в настоящее время с учетом статистических данных и внедрением неонатального скрининга необходимость раннего выполнения ТГСК не вызывает сомнений. Эволюция режимов кондиционирования в сторону минимизации рисков токсических осложнений и возникновения вторичных опухолей чревата персистенцией смешанного химеризма и недостаточным противоопухолевым потенциалом в случае наличия онкологического заболевания. Единые подходы к срокам проведения ТГСК при СН, выбору режима кондиционирования и профилактики реакции «трансплантат против хозяина» не стандартизированы.</p> <p><bold>Цель исследования:</bold> продемонстрировать становление подходов к ТГСК у детей с СН на базе одного Центра и провести ретроспективный анализ эффективности с оценкой отдаленных результатов.</p> <p><bold>Материалы и методы.</bold> На базе Российской детской клинической больницы за 23 года выполнено 9 ТГСК детям с СН.</p> <p><bold>Результаты.</bold> Общая 10-летняя выживаемость пациентов после ТГСК составила 0,67 (95% доверительный интервал 0,56–0,78). Было зарегистрировано 3 летальных исхода, ассоциированных с ТГСК, у большинства детей сохраняются смешанный химеризм и потребность в заместительной терапии иммуноглобулинами.</p> <p><bold>Заключение.</bold> С учетом современных тенденций оптимизированы методы проведения ТГСК при СН, позволившие улучшить результативность лечения.
