The incidence of tumor predisposition syndromes (TPS) in patients with MB of the SHH group is about 20%, and this is the highest rate among the other molecular genetic subgroups of MB. In addition, there is significant molecular genetic heterogeneity within the same group, which in turn contributes to variations in survival rates and prognosis.</p> <p><bold>Objective</bold> – to analyze the clinical, molecular, and genetic characteristics of TPS in patients with SHH group MB.</p> <p><bold>Materials and methods.</bold> This retrospective study included a cohort of 40 patients aged 0 to 18 years with SHH group MB who underwent molecular genetic diagnostic testing between 2018 and 2024 at the D. Rogachev National Medical Research Center. The MB group was determined using NanoString technology. To identify germline mutations, DNA isolated from peripheral blood was examined using the following methods: next-generation sequencing, whole genome sequencing, Sanger sequencing, multiplex ligation-dependent probe amplification. The amplification of the <italic>MYC </italic>and <italic>NMYC </italic>genes was detected using fluorescence <italic>in situ </italic>hybridization. This study did not require approval from the local Ethics Committee, as the analyzed data did not contain any personalized information. Consent was obtained for the peripheral blood testing for TPS and for the publication of the data. Statistical processing and analysis of the data obtained were carried out using the R package version 4.4.2. Overall survival was estimated using the Kaplan–Meier method, and the cumulative incidence of secondary tumors was estimated using the competing risk method. To calculate the overall survival, we used the time from the date of diagnosis to death, and for the cumulative incidence of secondary tumors, we used the time from the date of diagnosis to the development of a secondary tumor, while death was considered a competing event.</p> <p><bold>Results.</bold> Fifteen out of 40 patients were diagnosed with TPS. Gorlin syndrome was diagnosed in 7 people, Li–Fraumeni syndrome in 4 patients, there was one case each of Lynch syndrome and neurofibromatosis type 1, and a mutation in the <italic>ELP1 </italic>gene. One child had a combination of two syndromes (Gorlin and Lynch). The incidence of TPS in the analyzed cohort of 40 people was 37.5%. Thirteen people are alive, 2 patients died. The overall 4-year survival rate was 93% (the 95% confidence interval 82–100). A secondary tumor developed in 4 patients. The 4-year cumulative incidence of secondary tumors was 31% (the 95% confidence interval 6.4–61.0). All patients who developed a secondary tumor had received radiation therapy.</p> <p><bold>Conclusion.</bold> When detecting MB of the SHH, it is important to conduct molecular genetic testing for TPS, given their high frequency among patients with MB of this molecular subgroup. The data obtained will help optimize therapy (avoid radiation therapy for young children with confirmed Gorlin syndrome), as well as predict the course of the disease. In addition, genetic testing is necessary for follow-up of such patients and for counseling their parents regarding the familial forms of the described syndromes.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение.</bold> Медуллобластомы (МБ) подгруппы SHH cоставляют 25–30% всех МБ и связаны с аберрантным активированием сигнального внутриклеточного пути SHH. Частота встречаемости синдромов предрасположенности к опухолям (СПО) у пациентов с МБ подгруппы SHH составляет порядка 20%, и это самый высокий показатель среди остальных молекулярно-генетических групп MБ. Кроме того, в пределах одной группы отмечается выраженная молекулярно-генетическая гетерогенность, что, в свою очередь, обусловливает разные показатели выживаемости и прогноз. Цель – проанализировать клинические, молекулярно-генетические характеристики СПО у пациентов с МБ подгруппы SHH.</p> <p><bold>Материалы и методы.</bold> В ретроспективное исследование включена когорта из 40 пациентов с МБ подгруппы SHH в возрасте от 0 до 18 лет, которым за период с 2018 по 2024 г. проводилась молекулярно-генетическая диагностика в ФГБУ «НМИЦ ДГОИ им. Дмитрия Рогачева» Минздрава России. Определение подгруппы МБ выполнялось с использованием технологии NanoString. Для выявления герминальных мутаций ДНК, выделенная из периферической крови, была исследована с помощью следующих методов: секвенирование нового поколения (next-generation sequencing), полногеномное секвенирование (whole genome sequencing), секвенирование по Сэнгеру, мультиплексная лигазозависимая амплификация зондов (multiplex ligation-dependent probe amplification). Выявление амплификаций генов <italic>MYC </italic>и <italic>NMYC </italic>проводилось методом флуоресцентной <italic>in situ </italic>гибридизации. Данное исследование не требовало одобрения локального этического комитета в связи с тем, что проанализированные данные не содержали персонализированную информацию. Было получено согласие на исследование периферической крови на СПО и публикацию данных. Статистическая обработка и анализ полученных данных проводились с помощью пакета R версии 4.4.2. Общая выживаемость оценена по методу Каплана–Майера, а кумулятивная частота развития вторичной опухоли – по методу конкурирующих рисков. Для расчета общей выживаемости использовалось время от даты диагноза до смерти, для кумулятивной частоты развития вторичной опухоли – время от даты диагноза до развития вторичной опухоли, при этом конкурирующим событием считалась смерть.</p> <p><bold>Результаты.</bold> Установлено, что у 15 из 40 пациентов были выявлены СПО. Синдром Горлина диагностирован у 7 человек, синдром Ли–Фраумени – у 4, по 1 пациенту было с синдромом Линча, нейрофиброматозом 1-го типа и мутацией в гене <italic>ELP1</italic>, сочетание 2 синдромов (Горлина и Линча) было выявлено также у 1 ребенка. Частота встречаемости СПО в проанализированной когорте из 40 человек составила 37,5%, из них живы 13, умерли 2. Общая 4-летняя выживаемость составила 93% (95% доверительный интервал 82–100). Вторичная опухоль развилась у 4 пациентов. Четырехлетняя кумулятивная частота развития вторичной опухоли составила 31% (95% доверительный интервал 6,4–61,0). Все пациенты, развившие вторичную опухоль, получали лучевую терапию.</p> <p><bold>Заключение.</bold> При выявлении MБ подгруппы SHH важно проводить молекулярно-генетическое исследование на СПО, учитывая высокую частоту их встречаемости среди пациентов с данной молекулярной группой заболевания. Полученные данные помогут оптимизировать терапию (не проводить лучевую терапию детям младшего возраста при наличии подтвержденного синдрома Горлина), а также спрогнозировать течение заболевания.
